Consent Form and Description of Services
By hiring the agency’s services, the applicant acknowledges and agrees to the terms described below.
1. Nature of the Company
The contracted company is a travel agency that specializes in assistance with U.S. tourist visa applications.
The agency’s president is a registered Florida Notary / Immigration Forms Specialist, with extensive experience in the preparation and completion of immigration forms, including the DS-160.
The agency is not a law firm and does not provide legal advice, as prohibited by law. If legal advice, legal analysis, or legal representation is required, the applicant must consult a duly licensed immigration attorney.
2. Description of Services Provided
The contracted services include:
- Completion of the DS-160 form, based solely on the information and documents provided by the applicant;
- Preparation and delivery of a customized list of important documents to be presented on the day of the consular interview;
- Delivery of letter templates that may be required as supporting evidence, such as ties, financial support, or others, if applicable;
- Scheduling of the consular interview according to the applicant’s availability and preferred schedule;
- One online interview preparation meeting, scheduled for the day prior to the consular interview, including final guidance and interview simulation.
3. Submission of Documents by the Applicant
To ensure an organized process and compliance with timelines, the applicant agrees to:
- Submit all requested documents and information to the agency’s official email address within the established deadlines;
- Ensure that all submitted documents are clear, complete, and up to date.
Failure to submit documents on time may affect deadlines, scheduling, and the quality of the preparation.
4. Delivery of Documents and Materials
All forms, guidance materials, templates, and attachments related to the process will be delivered exclusively through a shared cloud folder, accessible to the applicant(s).
No physical printed documents will be provided.
5. Translations and Excluded Services
This service package does not include document translation, whether certified or non-certified.
If translation services are required, the agency may offer them through a separate quotation, subject to the applicant’s prior approval.
6. Responsibility for Information Provided
The applicant declares that all information provided is true, complete, and accurate, acknowledging that the accuracy of this information is essential for proper DS-160 completion and effective interview preparation.
7. Limitation of Liability
The applicant understands that:
- The agency has no affiliation with the U.S. government;
- The agency has no authority over visa approval or denial;
- Visa issuance is a decision made solely by the U.S. consular officer.
By proceeding with the service, the applicant confirms that they have read, understood, and agreed to all terms stated above.
